Summary
The Care Coordinator role combines care coordination and case management functions with general administrative responsibilities to support the effective delivery of Aged Care and Disability Services at Community Care TASMANIA. The role is responsible for managing a lower volume portfolio of Support at Home clients, providing high-quality, person-centred coordination of services, and supporting the organisation's operational administrative functions.
The Care Coordinator is a key first point of contact for Aged Care and Disability enquiries, responding to incoming phone calls, managing urgent or low-level issues, and triaging matters appropriately to Care Managers, Clinical staff, or other relevant teams. This is primarily an office-based role, requiring strong communication, organisational skills, and sound judgement to balance administrative duties with participant-focused care coordination.
Primary Responsibility
* Acting as a first response contact for Aged Care and Disability phone enquiries, including responding to urgent or low-level issues and triaging matters to appropriate internal teams.
* Managing a portfolio of Support at Home clients, ensuring services are coordinated in line with assessed needs, goals, preferences, and funding requirements.
* Supporting the development, implementation, and review of care and support plans in consultation with participants, families, and relevant stakeholders.
* Maintaining accurate and timely participant records, documentation, and case notes within the CRM/CMS, in line with organisational and legislative requirements.
* Supporting Care Managers with administrative components of care management, including documentation, consumables ordering, participant communication, service agreements, consent forms, and care plan reviews.
* Monitoring participant needs and identifying emerging issues or risks, escalating to Care Managers, Clinical Services, or Quality, Risk and Compliance as appropriate.
* Liaising with the rostering and scheduling teams to support the coordination of services and respond to routine service delivery enquiries.
* Assisting with day-to-day administrative duties of the Aged Care and Disability Team to support efficient delivery of services, including, participant enquiries, phone management, correspondence, and record keeping.
* Supporting internal communication through timely preparation and distribution of emails, letters, and other correspondence.
* Maintaining accurate records, filing, and data entry to support compliance, reporting, and service delivery.
* Contributing to continuous improvement initiatives related to administrative systems, processes, and care coordination practices.
* Maintaining good Work Health and Safety practices in accordance with Community Care Tasmania policies and procedures.
* Other duties as reasonably directed by the Manager Aged Care and Disability Services.
About you
* Demonstrated experience providing administrative support to a mobile workforce in a community or health-based setting.
* Demonstrated ability to work co-operatively as a member of a professional team.
* Ability to prioritise workload demands and cope calmly, effectively and efficiently under pressure.
* Proven capacity to maintain confidentiality and respect the privacy of Participants, managers and other employees.
* Proficient in the use of computer software and programs including but not limited to Microsoft 365, CRM systems, SharePoint, My Aged Care and Proda.
* Police check (less than 6 months or ability to gain)
* Current WWVP with NDIS endorsement (or ability to gain endorsement)
* Drivers license
* NDIS Workers Orientation Module.
