About the role
An exciting opportunity exists for a dynamic individual to join the South West Local Health Service Network in an exciting new role as SWLHSN Safer Together Program Coordinator.
This position will work across the South West Local Health Service Network but will be hosted by Western District Health Service. Our partners in the South West Local Health Service Network (SW LHSN) are: Casterton Memorial Hospital, Heywood Rural Health, Moyne Health Services, Portland District Health, South West Healthcare, Terang and Mortlake Health Service, Timboon and District Healthcare Service and Western District Health Service.
The South West Local Health Service Network is working collaboratively to drive improvements that benefit the wider community and our workforce. As a Network, we have four priority areas of focus: access, equity and flow; our workforce; safety and quality; and our shared services.
The Safer Together Program is funded to deliver work that aligns with the following 4 strategic priorities:
1. Implementing consistent evidence-based practice to ensure high-quality care and reduce harm.
2. Strengthening preventative measures to minimise the need for avoidable hospital admissions.
3. Ensuring safe medication practices to avoid harm and protect consumer health.
4. Focusing on necessary and effective interventions to improve consumer outcomes.
The role reports to the South West LHSN Director and will work closely with other team members to ensure the implementation and delivery of the STP functions.The position is hosted by Western District Health Service, with accountability to the South West LHSN Executive Committee via the SW LHSN Director.

The successful applicant will hold, or be able to obtain, a current National Criminal Police Check, Working with Children Check Clearance and NDIS Worker Screening Check. All successful applicants are to comply with WDHS immunisation and vaccination requirements.
