This is an exciting opportunity for an experienced Case Manager or passionate individual to make a difference in the lives of some of our most vulnerable. Your focus will be on providing flexible and responsive services tailored to the individual needs of clients, ensuring that they can continue to live in their homes safely and comfortably.
Day to day this role looks like:
As part of the Vulnerable Communities team, you will use your skills, passion and experience to oversee the Case Management of Aged Care clients in the Hunter area impacted due to hoarding and/or squalor.
* Facilitating a collaborative service approach to identify supportive solutions for people who experience Hoarding, Squalor or clutter
* Assessing, Coordinating and reviewing client care needs within their funding allocation.
* Coordinating a service response that is directed to ensuring housing is maintained for the older person and that their care needs are met so they can continue to live in the community
* Delivering creative strategies and solutions that deliver exceptional client outcomes.
* You will be responsible for ensuring the delivery of flexible and responsive services tailored to an individual's needs and interests, whilst also providing advocacy and referral services to clients.
* Collaborating with both government and nongovernment support agencies within the community care sector in order to achieve improved housing, health and well-being outcomes for clients.
* Having a genuine passion for care, balanced with a desire to maximise services and offer commercially sustainable case management in line with the organisation's strategy.
To be successful in this role you'll need
* Diligence, empathy, and drive, being motivated by your passion to assist others who are vulnerable members of the community
* Demonstrated experience in case management
* High level of communication skills and a "no judgement" approach
* Understanding of the complexities associated with situations of Hoarding and/or Squalor highly desirable
* Mental Health experience highly desirable (not essential)
* Relevant qualifications in Allied Health, Nursing, Social Science or related field is desirable (not essential)
* Highly developed planning and negotiation skills
* Demonstrated experience in managing the contractual requirements of government programs to ensure compliance, regulatory standards, and organisational policy and procedure (or open to learn)
* High degree of computer literacy with an ability to learn new software programs
* Registered and insured motor vehicle and a current Australian drivers' licence
