Support Young People as a Home-Based Care Worker
This role is ideal for individuals passionate about creating positive impacts in the lives of children and young people. As a Home-Based Care (HBC) Support Worker, you will work closely with clients and families within our Foster Care program to provide essential support.
You will collaborate with the HBC Case Manager and Coordinator to plan activities and provide emotional and physical well-being support to each child's unique needs. Your focus will be on ensuring their safety and happiness while working closely with other professionals to achieve successful outcomes.
* Job Description:
* Collaborate with the HBC Case Manager and Coordinator to develop and implement individualized care plans that meet the unique needs of each client.
* Provide emotional and physical support to clients and their families, including planning exciting activities and transporting them to appointments.
* Work closely with other professionals, such as social workers and teachers, to ensure comprehensive support services are provided.
* Required Skills and Qualifications:
* A Certificate IV in Child, Youth and Family Intervention (Residential and out of home care), or other similar tertiary qualifications.
* A current valid Victorian driver's licence and the ability to operate MacKillop passenger vehicles in accordance with all Victorian Road Safety Regulations.
* An understanding of Trauma and how best to work with children and young people in Out of Home Care.
