The Department The Alfred Specialty Medicine Program, is comprised of six clinical units including: Clinical Genetics and Genomics, Dermatology, Endocrine and Diabetes, Gastroenterology, Renal and Rheumatology. The program provides admitted services including multiday treatment (Ward 4WA) and Same Day Dialysis services and ambulatory care, at The Alfred, Caulfield and Sandringham campuses. The Alfred Specialty Medicine program runs a large outpatient service, with a focus on multidisciplinary specialty clinics with >42,000 patient consultations per year across The Alfred, Caulfield and Sandringham campuses. The inpatient component of the program admits >22,000 patients per year across the Gastroenterology, Renal, Endocrine and Diabetes and Rheumatology services. In addition, the program encompasses a world-leading research program, providing patients with access to the latest innovations in clinical care. Position Summary - Providing support and nursing care to patients during specialist clinic appointments - Supporting the multidisciplinary team to develop patient care plans - Undertaking clinical skills in line with clinic requirements (documenting patient observations and escalating clinical risk) - Coordinating effective and efficient discharge and follow up - Documenting clinical information utilising patients Electronic Medical Record - Acting as a role model to other members of the health care team - Participating in multidisciplinary discussions to support patient care where required Alfred Health nursing staff practice in accordance with the Alfred Health Nursing Professional Practice Framework. As a clinical nurse you are accountable for establishing and evaluating the patient’s plan of care, in partnership with the health consumer, their carers and other members of the multidisciplinary health care team.
