Child and Family Advocate
Support children and young people in Out-of-Home Care by developing individualized case plans prioritizing their needs.
Key Responsibilities:
* Develop and review case plans to meet therapeutic and cultural goals
* Cultivate relationships with families, caregivers, and agencies to ensure consistency
* Maintain accurate documentation and comply with relevant regulations
Qualifications and Experience:
* Diploma (or working towards) in community, youth, or social work
* Proven experience in case management and stakeholder engagement
* Knowledge of trauma-informed care practices and commitment to cultural safety
