Aged Care Assessor Job Description
As an Aged Care Assessor, you will be responsible for conducting Home Support Assessments through the My Aged Care platform. Using the Integrated Assessment Tool (IAT), you will identify and evaluate an older person's needs, goals, and preferences with a focus on wellness, reablement, and promoting independence.
The Tasmanian Health Service delivers the Aged Care Assessment Service (ACAS) South, which provides both clinical and non-clinical assessments as part of the Assessment and Case Management Service. Key Responsibilities:
* Conduct Home Support Assessments through the My Aged Care platform
* Use the Integrated Assessment Tool (IAT) to identify and evaluate an older person's needs, goals, and preferences
* Work collaboratively with the multidisciplinary team to deliver high-quality, person-centred care
Requirements:
* Well-developed understanding of and demonstrated ability to undertake the functions of an Aged Care Assessor
* Demonstrated understanding and knowledge of the needs of older clients and the concept of wellness and reablement
* Strong organisational skills and ability to meet deadlines

Other Information:
