This is a full‑time position located in Caboolture, QLD. Remuneration ranges from $45.47 to $48.93 per hour, plus superannuation, salary packaging and staff discounts.
About the Reconnect Program: The program supports young people aged 12–18 and their families or support networks to navigate challenging circumstances and build sustainable solutions. Reconnect offers counselling, mediation and practical support.

What You'll Be Doing:
* Provide case management, counselling and mediation to young people and families.
* Conduct holistic assessments and create client‑led support plans.
* Deliver evidence‑based therapeutic interventions.
* Assist young people to access accommodation, education, training and community supports.
* Facilitate group work, life‑skills programs and psycho‑education.
* Work flexibly, including outreach in the community and home settings.
* Develop strong connections with local schools, services and referral networks.
* Maintain accurate case notes, data entry and reporting requirements.
What You Bring:
* Experience supporting young people and/or families through case management.
* Strong knowledge of youth development, family systems, risk factors and intervention models.
* High‑level communication, engagement and de‑escalation skills.
* Ability to work autonomously and collaboratively within a multidisciplinary team.
* Confidence using Microsoft Office and case management systems.
* Essential: NDIS Worker Screening | Blue Card (Working with Children Check) | QLD Driver's Licence.
* Desirable: Human Services, Social Work, Psychology or related qualification.
