Registered Nurse - Geriatric Health Specialist
A unique opportunity to work as a Registered Nurse specializing in Geriatric Health. This role focuses on supporting older adults and their caregivers throughout the aging journey.
As a member of an Interprofessional Primary Care Team, you will conduct comprehensive geriatric assessments, develop individualized care plans, and support physicians in patient management.
* Active Practice Permit with CARNA
* Current BLS/CPR Certification
* 3-5 years' experience in nursing
* Experience in community-based care is an asset
* Experience in Geriatrics preferred
Key Responsibilities:
1. Conduct thorough nursing assessments based on individual concerns and complexity
2. Develop client-focused care plans tailored to meet individual needs
3. Collaborate with healthcare professionals to ensure high-quality care
4. Communicate effectively with clients, families, and healthcare teams
