The Department
The Mobile Assessment & Treatment Service (MATS), within the Health of Older People division, is a multidisciplinary team providing rapid clinical in-reach to residents of aged care facilities and the community at risk of emergency department presentation. The service caters primarily to frail, elderly patients with chronic conditions and age-related issues.
The Role
We are seeking a dynamic and experienced Registered Nurse to join our team. The successful candidate will be dedicated to enhancing health outcomes for frail elderly patients in residential and community settings. Responsibilities include:
1. Working collaboratively within a multidisciplinary team, practicing autonomously when appropriate.
2. Conducting comprehensive assessments to detect clinical deterioration.
3. Providing safe, patient-centered care, including administration of IV medications, wound management, urinary catheter care, and end-of-life support.
4. Evaluating and adjusting care plans in partnership with patients, their decision-makers, GPs, and residential staff.
5. Ensuring integrated care across hospital, GP, and residential care interfaces.
6. Maintaining excellent communication and documentation to foster effective stakeholder relationships.
This role suits RNs with significant experience in emergency, acute, general, or geriatric healthcare, passionate about delivering the best care for older persons.
Qualifications and Experience
Essential:
* Registration as a Registered Nurse with AHPRA
* Relevant clinical experience (preferably 5+ years post-graduation)
* Ability to work positively and flexibly with internal and external services
* Strong interpersonal, written, and verbal communication skills
* Commitment to ongoing professional development
* Current Victorian Driver's Licence
Desirable:
* Postgraduate qualifications in gerontology or palliative care nursing
* Experience in community wound assessment and management
* Credentialing in male/female IDC and SPC catheterisation
* Recent clinical experience in general medicine, aged care, or emergency nursing
