Key Responsibilities
* Provide wound, catheter, stoma, and bowel care to clients in their homes
* Administer medications and injections as prescribed by healthcare professionals
* Manage chronic diseases and conditions in clients' homes, utilizing clinical assessment, decision-making, and problem-solving skills to deliver high-quality care
* Communicate effectively with clients, families, other clinicians, and the healthcare team to ensure seamless care coordination
Requirements
* Degree or postgraduate qualification in Nursing
* Minimum 2 years of experience in general nursing, preferably in community care settings
* Current Registration with the AHPRA, with zero notations
* Genuine interest in community care and aged populations, with a strong desire to make a positive impact on people's lives
* Compassion, enthusiasm, and a friendly approach to interacting with clients and colleagues
* Strong experience in wound and catheter care, with a focus on delivering high-quality outcomes
* Advanced clinical assessment skills for chronic disease management, with the ability to think critically and make informed decisions
* Excellent verbal and written communication skills, with the ability to communicate effectively with diverse stakeholders
