About The Role
Looking for a role where you actually see the impact of your work every day? Join us as an Occupational Therapist in the Transitional Aged Care Program (TACP) based in Taree, and help older people get back to living well at home after a hospital stay.
This temporary part-time role is perfect if you enjoy variety, autonomy and genuine connection with your clients. You'll be out in the community visiting people in their homes, problem-solving on the spot, and putting practical strategies in place that help them feel safe, confident and independent again.
You'll be part of a small, supportive multidisciplinary team that values your ideas and clinical input. Together, you'll set meaningful goals, deliver short-term reablement, and walk alongside clients and their families with warmth, respect and dignity.
In This Role, You Will
Provide community-based OT to older adults following hospital discharge
Complete in-home functional assessments and develop tailored, person-centred treatment plans
Recommend and coordinate equipment and simple home modifications to maximise safety and independence
Work closely with multidisciplinary colleagues to ensure smooth, safe transitions of care
Support clients to improve function, build confidence and achieve the goals that matter most to them
Contribute to a high-quality, Commonwealth-funded TACP program that truly makes a difference
About You
You enjoy working with older people and you're good at putting them at ease. You're practical, down-to-earth and able to adapt quickly to whatever each home visit brings. You communicate clearly, build strong rapport with clients and families, and you're confident making clinical decisions in the community with a supportive team behind you.
This role suits both early-career and experienced OTs. Most importantly, you bring:
AHPRA registration as an Occupational Therapist
An interest in aged care, rehabilitation and community-based practice
Strong assessment, communication and documentation skills
Confidence working independently in client homes while staying connected with the team
A current driver's licence and reliable transport
