Job Title: Clinical Care Coordinator
Clinical care coordinators play a pivotal role in ensuring residents receive exceptional care, while fostering an environment that respects their unique identities and experiences. They lead clinical and care teams to deliver comprehensive and specialized nursing care, tailored to each resident's needs.
Key responsibilities include:
* Providing comprehensive clinical and specialized nursing care is delivered, documented, and tailored to each resident's needs by our clinical staff.
* Prioritizing residents and their families in decision-making, considering their preferences, independence, dignity, and identified care-related risks.
* Overseeing and conducting all scheduled AN-ACC reappraisals, including mandatory and voluntary assessments, as required.
* Holding Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting, care planning, and addressing patient concerns promptly.
* Conducting routine clinical assessments to address identified gaps in care.
About the Ideal Candidate:
* Currently registered as a Nurse with AHPRA, with no restrictions.
* Demonstrated senior management experience in the aged care sector.
* In-depth knowledge of the aged care industry, with a keen eye for emerging trends and best practices.
* Proficiency in managing human resource matters, complemented by strong negotiation skills.
* Skilled in utilizing various information technology tools and software, including Word and Excel, with a quick ability to adapt to new skills as required.
