About the role:The Geriatric Evaluation and Rapid Intervention (GERI) team at Rockhampton Hospital focuses on early assessment of frail older patients, aimed at improving clinical and disposition decision making. Within the Emergency Department (ED), the GERI team will function as an interdisciplinary team to provide the following key functions:
Geriatric assessment and intervention in the Emergency Department for frail patients over 50 years (indigenous) or over 65 years (non-indigenous).Referral to appropriate care services.Provide a follow up service for patients that present and are discharged from the Emergency Department.Facilitate admission with inpatient teams and provide follow up specialist geriatric consultations where required.Provide fast-tracked home assessment by relevant allied health professionals.Implement an ongoing training and development program to provide capability uplift for clinicians in acute facilities and Residential Aged Care Facilities (RACFs) across Central Queensland.About you:Appointment to this position requires mandatory possession of Bachelor's degree in Physiotherapy from a recognised tertiary institution and registration as a Physiotherapist with the Australian Health Practitioner Regulation Agency (AHPRA).Whilst not mandatory, post graduate qualifications in a relevant practice area is highly desirable.What we offer:Total remuneration value of up to $157,359 p.a. is comprised of:
