What You'll Be Doing
Work as a Level 1/2 Social Worker within the South Eastern Sydney Local Health District (SESLHD) SOS team. Support older adults and younger people with disability who are at risk of hospital admission, delivering high‐quality care to help them remain safely at home. Provide assessment, case management, advocacy and coordination for clients with complex needs, while contributing expertise to a respectful and inclusive team environment.
Selection Criteria
* Unrestricted NSW driver's licence.
* Bachelor of Social Work and eligibility for membership of the Australian Association of Social Workers (AASW).
* Demonstrated understanding of aged care issues and younger clients with severe and chronic disability in a community setting.
* Demonstrated organisational skills and ability to manage a demanding caseload.
* Demonstrated high level communication, report‐writing, and computer skills.
* Demonstrated ability to work independently and as part of a multidisciplinary team with minimal direct supervision.
* Demonstrated commitment to ongoing quality improvement, evidence‐based practice, and ongoing clinical and professional learning.
