Clinical Coordinator - Lara Precinct and Regional Services (RPN4)
Based at Western Plains Correctional Centre (WPCC) 1150 Bacchus Marsh Road, Lara 3212.
* Lara Precinct comprising of Western Plains, Barwon Marngoneet and Karreenga Correctional Centres
* Regional Location comprising of Hopkins, Langi Kal Kal, Beechworth, Loddon & Middelton Correctional Centres
The Clinical Coordinator – Lara Precinct and Regional Services ensures efficient triage, assessment and coordination of mental health care for consumers referred to Forensicare at the Lara Precinct and in Regional prisons.
The Clinical Coordinator liaises closely with primary mental health service providers across the above locations to coordinate care and supports transition to other prison locations as relevant. The Clinical Coordinator provides specialist mental health consultation to relevant stakeholders in order to promote clients' mental health recovery and coping in custodial environment. As the senior Forensicare nurse at this location, the Clinical Coordinator is also responsible for providing clinical leadership, supervision and support to Forensicare's Lara Precinct nursing staff, and ensuring a high standard of interagency engagement with stakeholders
The role of the Clinical Coordinator at WPCC:
* Receives and appropriately triage all referrals to Forensicare services at the Lara Precinct and Regional locations
* Applies to practice an advanced understanding of the acceptance criteria of the Lara Precinct and Regional Prisons and understands discharge processes and requirements to efficiently support prisoner transfers and dischargers.
* Prioritises waitlists and schedules appointments according to risk, acuity, distress and impairment.
* Undertakes RAPID checks – Redevelopment of Acute and Psychiatric Information Directions (RAPID) is a comprehensive data base of all persons who access public mental health systems within Victoria.
* Role model and promote effective and collaborative working relationships with Forensicare staff and stakeholders
