Build Participant capacity to manage their own NDIS plan implementation by helping them to understand funding categories in their plan.
Assisted participants to find and choose services that helped them build capacity to achieve their goals.
Assisted the participants for NDIS plan review meetings by liaising with NDIA delegates and their service providers for supporting evidence.
Assisted Participants to manage crises such as finance, health, housing and social isolation by liaising with relevant agencies, community and government supports.
Completed support coordination reports and risk assessments prior to undertaking a new manual task.
Implemented organizational development plan for quality
Understand and provide appropriate support to Participants with low and high needs who demonstrate a range of challenging behaviors
• Assisting Participants to identify preferred supports relating to daily living, community support, and empowerment
• Work under general guidance from the person seeking support, and their family, within clearly defined guidelines
• Perform administrative duties and maintain accurate case records
• Participate in case reviews to determine the needs and wellbeing of
the Participant
• Attend Team Meetings and other meetings as required
• Accurately record daily activities
