If you are ready to make a meaningful impact in the lives of others, we would love to hear from you.What You'll Be DoingThe Clinical Nurse Specialist Grade 2 End of Life Care Coordinator (CNS2 EoLCC) will provide safe, compassionate, quality, person centered specialist care to support the delivery of End-of-Life Care (EoLC) across South Western Sydney Local Health District (SWSLHD). The position will collaborate with patients, families, carers, and a multidisciplinary team to achieve specific goals and outcomes. The role also includes providing telehealth consultations to SWSLHD sites that lack EoLC services, especially during weekends. The CNS2 EoLCC will review current practice, assist in introducing evidence-based models of care such as the last days of life toolkit, Advance Care Planning (ACP) and Voluntary Assisted Dying (VAD).Where You'll Be WorkingBankstown-Lidcombe Hospital is located in the city of Bankstown and is in one of the most multicultural local government areas (LGA) in Australia.
