Our Journey
We are seeking an experienced Social Worker Grade 3 to join our Cystic Fibrosis Unit on a part-time temporary basis until July 2024. (With strong possibility of role extension)
Social Workers at Monash Medical Centre sit within our Allied Health Team, providing specialised services to infant, paediatric, adolescent and Adult Cystic Fibrosis patients and their families. Cystic Fibrosis Social work services are provided across inpatient, outpatient and emergency settings including Paediatric ICU, Neonatal Intensive Care Unit; ambulatory care services such as the Department of Respiratory medicine in both children and adult settings.
A Day in the Life
As the Grade 3 Cystic Fibrosis Social Worker at Monash Health, you will collaborate with colleagues across a number of programs, the paediatric/adult acute, subacute and community sectors to foster clinical excellence and consistency of practice within the service. You will provide social work services primarily to patients with Cystic Fibrosis and their families/carers, however maybe required to work in other areas of the hospital at times. In this role you will work very closely within the expert, supportive multi-disciplinary Cystic Fibrosis team. It is a fast paced role with excellent learning opportunities to develop your clinical skills. There is scope to develop or use your quality improvement, teaching and/or research skills.
You will be required to participate in the after-hours on call roster at Monash Health and may be expected to work across sites within Monash Health as required.
What you will bring:
1. You will have a good knowledge of resources and understanding of the assessment and intervention frameworks used to guide practice including the Children’s Youth and Families Act and appropriate referral networks.
2. You will be able to demonstrate skills in psycho-social assessments, problem solving, communication and discharge planning.
3. You will be skilled at supporting families adjusting to their child’s illness and able to provide grief and bereavement support as required across all generations
4. You will develop and maintain strong links with universities and your Monash Health social work colleagues and supervise students and other staff as required.
5. You will be a dynamic, innovative and flexible practitioner and will receive regular supervision from highly experienced social work supervisors with opportunities for ongoing professional development.
What you will need:
6. A relevant social work qualification
7. Eligible for AASW membership
8. Experience preferably in an acute health care setting and/or with children and families of > 5 years
