Overview
We are establishing a 12-month casual pool for Medical Consultants to join Central Adelaide Local Health Network's Integrated Care division. Successful applicants may be offered casual opportunities to support services such as Hospital in the Home (HITH) or broader Integrated Care programs, depending on service needs and candidate suitability. This is an opportunity to work flexibly across innovative models of care transforming how health services are delivered in South Australia.
Central Adelaide Local Health Network's Integrated Care division provides services aimed at reducing hospital admissions, shortening acute hospital stays, and bridging the gap between acute hospitals and community-based care, with a focus on patient-centred, timely, and safe clinical interventions.
Roles and Responsibilities
As a Medical Consultant in Hospital in the Home (HITH): deliver acute, inpatient-equivalent care to patients in their homes or preferred non-hospital settings. Use hospital-grade technologies, medications, and safety and quality standards. Collaborate with emergency departments, acute and sub-acute services, community providers, and virtual care platforms. Manage patients requiring rapid, time-limited interventions, support hospital avoidance strategies, and ensure patients receive the right care in the right place. HITH operates seven days a week, including public holidays, and involves participation in a remote on-call roster.
As a Medical Consultant in Integrated Care (more broadly): provide rapid assessment, clinical advice, virtual care, and short-term interventions to support early discharge from hospital, avoid unnecessary presentations, and bridge care between primary and tertiary settings. Work as part of a multidisciplinary team, contribute to direct patient care, clinical teaching, continuous quality improvement, and the development of innovative community-based service models.
Both roles offer opportunities to contribute to contemporary healthcare delivery, supporting patients to achieve better outcomes and strengthening the efficiency of the broader health system.
