Care Navigators Play Key Role in Recovery Support
Improving patient mental health outcomes and preventing readmissions are critical goals for care navigators. By providing comprehensive, recovery-oriented outpatient/outreach services following discharge from an inpatient setting, these professionals can make a significant difference.
Key Responsibilities:
* Identify patients who could benefit from care navigation and develop a plan to support them, ensuring a seamless transition to the least restrictive care environment.
* Assess patients' needs upon admission and work with their healthcare team to create a personalized care plan that addresses their unique challenges and circumstances.
* Support patients as they access ongoing outpatient/outreach care, helping them navigate the healthcare system and connect with resources that meet their needs.
* Collaborate with treating psychiatrists and other members of the multidisciplinary care team to coordinate and deliver patient-centered care.
Qualifications:
* At least 5 years of experience in clinical mental health settings, with a strong understanding of mental health conditions, pathways, and referral processes.
* Excellent assessment, communication, and organizational skills, with the ability to develop and implement effective care plans and manage complex patient relationships.
* A current AHPRA registration as a Registered Nurse, Mental Health Nurse, or a Mental Health Allied Health professional is required.
