About the role:
As a Social Connect Support Worker in our Day Programs at Gateways, you'll play a vital role in supporting people with disabilities to live as independently and confidently as possible. Working alongside participants, you'll help design and deliver programs that build life skills, foster social connections, and promote engagement in meaningful community activities. You'll work rostered shifts, generally between 9:00am and 3:00pm, collaborating closely with Team Leaders, colleagues, managers, families, community groups and other service providers to ensure high quality, person-centred support.
Duties will include:
* Encourage independence by supporting individuals to develop skills and achieve personal goals.
* Build meaningful relationships and provide person-centred support based on respect, dignity, and empathy.
* Support participants to engage in community-based programs and activities that promote inclusion and wellbeing.
* Maintain accurate records of activities, progress, and incidents in line with organisational procedures.
* Provide safe and reliable transport to and from community activities using Gateways vehicles.
* Contribute to the ongoing monitoring and evaluation of service performance and participant outcomes.
* Liaise effectively with internal and external stakeholders to ensure responsive, coordinated support.

Week 1 - Monday, Tuesday, Wednesday 9am - 3pm
Week 2 - Monday, Tuesday, Wednesday 9am - 3pm
Position hours per fortnight, maternity leave cover until August 2026
Week 1 - Thursday, Friday, Monday 9am - 3pm
Week 2 - Thursday, Friday, Monday, Wednesday 9am - 3pm

Week 2 - Thursday, Wednesday 9am - 3pm

What you will bring to the role:
* Excellent communication skills, with the ability to problem solve
* Strong emotional intelligence and interpersonal skills
* Ability to accurately record case notes.
* Ability to work cohesively in a team environment.
* A passion for supporting people to live their best lives
* Current CPR, First Aid, Working with Children Check, NDIS Check & completion of the NDIS Workers Orientation Module (or willingness to obtain)
* Victorian drivers licence
