About the Role
This role is responsible for ensuring quality care and support is delivered in a customer-centric way to enable more older Australians to remain living at home safely.
As Care Manager, you will be responsible for the planning and oversight of the scheduling and delivery of timely care and supports that meet customer goals and deliver exceptional service.

Main Responsibilities
* Plan and oversee the scheduling of support workers and third-party suppliers to deliver quality care and services
* Work collaboratively with the Service Manager to undertake and ensure timely recruitment and effective performance management of staff
* Develop and review Care Plans in consultation with customers and their families
* Accurately record and monitor individual customers' budgets to reflect their care plans
* Regularly review customers' Care Plans.
Requirements and Qualifications
* A minimum 12 months experience ideally in aged support services
* A highly professional attitude and ability to act with compassion
* Exceptional time management skills
* Excellent administrative and computer skills
* Excellent communication skills
Desirable Skills and Qualifications
* Social, Home & Community Services, Aged Care, Disability, Health Industry or related relevant experience
* A current driver's license and safe driving record
* A willingness to travel to visit customers
