Job Overview
We are looking for experienced registered nurses to work as part of our Integrated Care Teams. You will participate in assessment, implementation, and evaluation of care to patients in their own homes and help develop the Primary Health Care Team.

Person specification
* Qualifications – Essential NMC registration; evidence of ongoing continuing professional development.

* Experience – Essential Previous experience in community nursing.

* Driving Car driver with access to a car.
