Overview 
The Acute to Aged Related Services (AARCS) team provides specialised care for older patients in acute inpatient settings who have complex care needs. This service delivers patient-centred, comprehensive assessment, care planning, and coordination to enable safe and timely discharges. You will be part of a collaborative, supportive culture that advocates for older people and their families, ensuring their needs remain at the centre of care. 
About the Role 
As a Clinical Nurse Specialist 2 in AARCS, you will provide advanced clinical care, leadership, and expertise in managing complex cases. Your responsibilities include delivering the first face-to-face assessment within one working day of referral, then working with families, carers, and service providers to develop safe discharge plans. You’ll balance competing priorities, manage patients exhibiting challenging behaviours, and advocate for older patients while guiding and supporting your colleagues. Each day will involve clinical leadership, applying advanced assessment and case management skills, and contributing to improving outcomes for older people. 
Responsibilities 
- Provide advanced clinical care for older patients and lead complex case management. 
- Deliver the first face-to-face assessment within one working day of referral. 
- Collaborate with families, carers, and service providers to develop safe discharge plans. 
- Balance priorities, manage challenging behaviours, and advocate for patients’ needs. 
- Support and mentor colleagues in clinical decision-making and care planning. 
Qualifications and Requirements 
- Current registration as a Registered Nurse with AHPRA. 
- Post-registration qualifications and experience appropriate to the CNS2 role.
