About the Opportunity
* Collaborate with other healthcare team members to deliver timely and effective care, preventing deterioration and reducing hospital presentations and admissions.
* Develop your Scope of Practice into Emergency and Acute Care, Mental Health, and Chronic and Complex Care.
Key Responsibilities:
1. Enhance healthcare services for older persons by addressing existing service gaps within Models of Care.
2. Improve health outcomes for older persons through provision of specialist aged healthcare services.
3. Collaborate with healthcare teams to prevent decline, reduce hospital attendances, and admissions.
4. Expand your Scope of Practice into Emergency and Acute Care, Mental Health, and Chronic and Complex Care.
Requirements:
* Australian citizenship or permanent residency status.
* Independence, legality, and current residence in Australia.
