You will work collaboratively with clients, families, carers, and partner agencies to provide comprehensive biopsychosocial assessments and deliver recovery-oriented, evidence-based treatment.You’ll also participate in a duty roster that includes providing urgent assessments and interventions across various settings—including clients' homes, emergency departments, medical wards, and aged/youth services.Key ResponsibilitiesConduct comprehensive biopsychosocial and risk assessmentsDevelop and implement recovery-focused care plansWork collaboratively within a multidisciplinary team (MDT)Support clients with medication management and physical health monitoringEngage with community services including NDIS, private clinicians, and GPsContribute to tribunal reporting and discharge planningParticipate in weekend duty rosters and emergency responseMaintain clinical records and data reporting in accordance with policyLocationYou will be working in a vibrant regional city approximately 300km northwest of Melbourne with a population of 20,000. This close-knit community is a hub for agriculture, especially grain and sheep farming.
