Aboriginal Health Worker / Practitioner – Diabetes Alliance Program Plus – Identified

Remuneration: $65,012 - $93,971 per annum *plus superannuation + salary packaging*; Location: Wisteria House, James Fletcher Campus, Awabakal Country; Employment Type: Temporary Full Time – Until December 2027; Position Classification: Aboriginal Health Worker / Practitioner; Requisition ID: REQ ; Applications Close: Sunday 18th January 2026.


About the Role

This position is identified for Aboriginal or Torres Strait Islander applicants under the Genuine Occupational Qualification criteria. The Aboriginal Health Worker/Practitioner will deliver culturally appropriate diabetes care across diverse communities in the Hunter New England region, supporting patients within Aboriginal Medical Services and GP practices while travelling throughout the region.


Responsibilities

* Provide care within scope of practice, guided by the HNE Health Aboriginal Health Workers and Practitioners Framework.
* Engage with local Aboriginal communities to design and implement culturally safe health strategies.
* Build partnerships with AMS, ACCHOs, LALCs, and primary care providers.
* Support individuals and families to understand clinical interventions and make informed health decisions.
* Collaborate within a multidisciplinary team and lead the Aboriginal Reference Group to ensure cultural safety in program priorities.
* Manage essential administrative tasks, including data collection and documentation.


Qualifications & Requirements

Aboriginal Health Worker:
- Holds a minimum Certificate III qualification in Aboriginal Primary Health Care or a minimum Certificate III health qualification in the area of care relevant to this role.

Aboriginal Health Practitioner:
- Holds a Certificate IV in Aboriginal Primary Health Care (Practice) and a current authority to practice as an Aboriginal Health Practitioner with the AHPRA.

Additional Requirements:
- Eligible to drive in NSW and able to travel as required.

Determination 1 of 23 Aboriginal Health Worker classification – recognition of prior service may influence commencement rate of pay.
