Role Summary
As a Clinical Care Coordinator, you will deliver high-quality patient care and support services in a General Practice setting.

Key Responsibilities:
* Deliver clinical-based nursing care in a General Practice under the supervision of General Practitioners.
* Conduct health assessments and develop care plans tailored to individual patient needs.
* Assist with minor procedures, including dressings and wound care.
* Manage chronic diseases, administer immunizations, and perform ECGs and spirometry tests.
* Provide education and health promotion services to patients and their families.
* Collaborate with healthcare professionals to ensure seamless patient care.
* Contribute to policy development and accreditation processes.
Requirements:
* Bachelor's degree in Nursing or equivalent qualification.
* Current registration with the relevant Nursing Board.
* Strong communication and interpersonal skills.
