Overview

Rural Warwickshire North Primary Care Network (PCN) is inviting applications for a PCN Clinical Pharmacist. This role is central to delivering high-quality, safe, and effective medicines management across our practices. The closing date is 01 October 2025.

We are delighted to offer an exciting opportunity for a Clinical Pharmacist to join Rural Warwickshire North Primary Care Network (PCN) as part of our growing multidisciplinary team. This role is central to delivering high-quality, safe, and effective medicines management across our practices.

As a PCN Clinical Pharmacist, you will work closely with GPs, nurses, and other healthcare professionals to optimise prescribing, support long-term condition management, and improve patient outcomes. You will conduct structured medication reviews, provide expert clinical advice, and play a key role in improving patient safety and reducing health inequalities.

Main duties of the job

The Clinical Pharmacist will work as part of the PCN multidisciplinary team to optimise medicines use and improve patient care. Key responsibilities include conducting structured medication reviews, supporting safe repeat prescribing systems, providing expert clinical advice on medicines management, and ensuring cost-effective, evidence-based prescribing. The postholder will liaise with GPs, practice staff, community pharmacies, and secondary care to improve continuity of care, support patients in understanding and managing their medicines, and contribute to wider PCN priorities such as long-term condition management, population health, and quality improvement initiatives.

We are a Primary Care Network (PCN) of five practices working together with a range of local providers, including primary care, community services, social care, and the voluntary sector, to offer more personalized, coordinated health and social care to their local populations.

Our PCN comprises 35k+ patients over the five practices, making us large enough to provide resilience and support the development of integrated teams.

- Protect, improve, and extend local General Practice through greater organisational scale to provide convenient high quality, comprehensive and integrated services.
- Enhance the skills and flexibility of the General Practice workforce to meet population and service needs with programmes of continuing professional development.
- Work and collaborate with others to innovate in service provision, building on new technologies and recognising lifestyle choices of service users in the way in which they wish to access care.
- Ensure that all we do is valuable, both for the taxpayer and service user and for those investing their time and money into the PCN.
- Operate ethically, with accountability, probity, and fairness at the heart of the organisation.

Job responsibilities

- Undertake structured medication reviews (SMRs) for patients with complex polypharmacy, long-term conditions, frailty, or high-risk medicines.
- Support safe and effective repeat prescribing systems within practices.
- Provide advice and support on medicines management, prescribing queries, and cost-effective prescribing.
- Contribute to the management of long-term conditions by optimising medicines use and supporting self-care.
- Conduct medicines reconciliation following hospital discharge and ensure appropriate follow-up.
- Liaise with community pharmacists, hospitals, and other providers to ensure continuity of care.
- Provide patient-facing support, educating patients on secure and effective use of medicines.
- Review prescribing in line with local and national guidelines to ensure safe, effective, and efficient use of NHS resources.
- Support practices to meet QOF, IIF, DES, and other contractual requirements through high-quality prescribing.
- Undertake audits, contribute to clinical governance, and lead quality improvement initiatives related to medicines safety.
- Work towards or maintain Independent Prescriber status and use prescribing skills to support patient care.
- Participate in appraisal, CPD, and mandatory training.
- Contribute to the education and training of other healthcare professionals where appropriate.
- Work collaboratively as part of the PCN multidisciplinary team.
- Provide support and advice to GPs, nurses, and ARRS staff on medicines-related issues.
- Attend PCN and practice meetings, contributing to service development.
- Engage with population health management initiatives and help address health inequalities through medicines optimisation.

Person Specification

Qualifications

- Registered Pharmacist with the General Pharmaceutical Council (GPhC).
- Evidence of continuing professional development.
- Strong clinical knowledge and understanding of medicines management.
- Experience working with patients and healthcare professionals.
- Excellent communication, organisational, and IT skills.
- Ability to work independently and as part of a multidisciplinary team.
- Patient-centred, flexible, reliable, and motivated.
- Independent Prescriber qualification (or working towards it).
- Experience of working in primary care or within a PCN.
- Experience in conducting structured medication reviews.
- Knowledge of EMIS and NHS prescribing systems.
- Involvement in service development, audit, or quality improvement.
- Completed CPPE Pathway or willing to enrol on it.
