We are looking for an experienced and self-motivated Registered Nurse to join our dedicated team in a permanent or part-time capacity, working 2–5 days per week. Responsibilities: Providing comprehensive nursing care with a focus on chronic disease management Assisting GPs with the preparation, delivery, and review of care plans, team care arrangements, and health assessments Delivering health coaching and supporting patients in achieving their health goals Liaising with allied health professionals to ensure coordinated patient care Managing clinical systems to maintain effective patient recalls Ordering vaccines and managing cold chain compliance Ordering and managing clinical supplies Assisting doctors with clinical procedures Collaborating closely with admin staff, GPs, and allied health providers About You: Current AHPRA registration as a Registered Nurse Experience in chronic disease management, health coaching, and behaviour change (highly regarded) Excellent communication skills and ability to work with diverse populations Computer literacy and familiarity with clinical software Passion for patient care and continuous improvement Able to thrive in a busy, team-oriented environment Apply today to be part of a team that truly values your expertise and contribution.
