Responsibilities
Clinically guide a specialist Social Work service to patients, clients, consumers and their family/carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct.
Provide a high-quality specialist Social Work service for Homeless Health, supporting people experiencing homelessness to secure and/or maintain stable accommodation through improved care planning and service coordination within health and between health and the broader community sector and the appropriate use of limited brokerage funds.
Strengthen partnerships and collaboration between health and the community sector; work closely with the NGO responsible for administering brokerage funds; lead clinical service improvement across the Illawarra Shoalhaven Health District in the area of Homelessness Health.
A Little About Us
Shoalhaven Memorial Hospital, located on Yuin Country, is the main acute care hospital for the Shoalhaven region, providing emergency care, medical, surgical and orthopaedic services. The hospital has an Intensive Care Unit focusing on high dependency and coronary patients, Children's Ward and Maternity Unit, as well as a purpose-built regional Cancer Care Centre and sub-acute Mental Health Unit. There is a significant redevelopment of the hospital underway with a focus on career development.

Requirements
Applicable vaccination requirements for a Category A role
Working With Children Check (WWCC)
Social Work degree and eligibility for the Australian Association of Social Workers (AASW)
Valid NSW Drivers Licence
