Your opportunity: * To deliver high level Occupational Therapy clinical services and professional expertise within the Integrated Care Services, Bundaberg. * Work autonomously within the multidisciplinary Integrated Care team to manage, coordinate and provide optimal, responsive and flexible Occupational Therapy services in acute and community settings. * The position will focus on preventing inappropriate and avoidable admissions by supporting the needs of the patient, their family and significant others through the transition from an acute care environment to the home setting.
