The role of Practice Nurse is crucial to our healthcare team, delivering culturally safe care and supporting chronic disease management.
We are seeking a skilled Registered Nurse with 3 years' experience in primary health care, community health, and chronic disease management. The ideal candidate has knowledge of Aboriginal and Torres Strait Islander health and is committed to culturally safe practice.
Responsibilities include managing recalls and follow-ups for continuity of care, working closely with General Practitioners, Aboriginal Health Workers, and allied health staff to improve access to care.
Key requirements include registration as a Registered Nurse with AHPRA, competence in immunisation administration and recall management, and knowledge of cold chain management and infection prevention and control.
