Job Overview
As the HACC PYP (Home and Community Care Program for Younger People) & SFC (Support for Carers) Lead Program Coordinator, you will be responsible for the delivery, performance, and quality of the HACC PYP and Carer Services within designated local government areas. With a strong operational focus, the position also contributes to the development of strategic initiatives and service planning. You will provide leadership to a team of Coordinators and play a key part in shaping and delivering services that respond to consumer needs, identifying emerging service requirements and contributing to continuous service improvement.

Your Skills and Experience
Tertiary qualifications in either Disability, Social Work, Health, Welfare, Community Services Development or equivalent.
Demonstrated understanding of privacy and confidentiality obligations.
Experience or knowledge regarding the provision of care to people with a disability and/or medical condition, younger people, and support for their carers.
High level engagement, care planning and assessment skills.
The ability to work independently and as part of a team.
Demonstrated experience in performing community and/or health assessments using a strengths-based approach and development of associated care plans.
Excellent interpersonal and health literacy skills.
