The MATS patient cohorts are the frail and elderly, or those with severe disability in residential settings. Often MATS patients have a range of chronic conditions, and conditions of aging.The RoleMATS is looking for a dynamic and experienced Registered Nurse to join our team. The successful applicant will be motivated to optimise the health experiences of frail elderly patients living in residential care and the community setting.The Outreach Nurse is responsible for:Working within a collaborative multidisciplinary team framework to ensure best care, whilst at times practising autonomously when providing that careConducting comprehensive and holistic systems assessments, with astute ability to detect and identify clinical deterioration.Delivery of safe patient centred care as determined by the multidisciplinary team, and overseen by the Geriatric Consultants.
