About the role
We are seeking a motivated and experienced Customer Journey Team Leader to lead our admissions and care planning team, supporting seamless transitions and quality care outcomes.
In this leadership role, you will oversee the coordination and delivery of clinical care planning processes, including new admissions, transition reviews, and care plan evaluations. You will ensure all activities meet compliance, funding, and quality standards while working collaboratively with clinical and support teams across CCH.
Duties and shifts below:
Lead and support the Customer Journey Team across all CCH sites
Allocate and oversee care documentation including new admissions, reviews, and evaluations
Ensure person-centred, evidence-based clinical pathways are consistently delivered
Lead, guide and manage Personal Care Workers and Enrolled Nurses and Registered Nurses
Have a commitment to continuous improvement, WHS and risk management
Demonstrate a solid understanding of the Residential Aged Care Accreditation Standards
Facilitate onboarding and training for team members as needed
Requirements for the role.
Bachelor of Nursing Qualification(Essential)
A National Police Clearance
The successful candidate will have demonstrated experience in a similar role and will have experience in the Aged Care industry.
