Responsibilities will include assessment, care coordination, intervention, liaison, advocacy, and review for patients referred to the service. The caseload is varied and may include neurological, orthopedic, amputee, chronic illness, and palliative care cases.

The successful candidate will have an approved degree in Social Work, including eligibility for membership with the AASW, as well as experience working in a hospital environment. Strong communication, conflict resolution, and time management skills are essential, along with the ability to work effectively within a multidisciplinary team.

Required Skills and Qualifications

* An approved Degree in Social Work that includes eligibility for membership with the AASW;

* Hospital experience;

* Demonstrated conflict resolution skills;

* Ability to prioritise caseload and work within timeframes;

* Demonstrated ability to effectively manage both clinical workload and administrative duties;

* Demonstrated ability to work as a member of a multidisciplinary team;

* Demonstrated commitment to professional development and continuous improvement;

* Knowledge on a variety of health related conditions;

* Ensure patient/consumer care is delivered within professional, organisational, legal and ethical boundaries and reflects evidence based, best practice knowledge;

* Intermediate to advanced computer skills.
