Pearl Home Care Gippsland/Cardinia is seeking an exceptional Case Manager for our Cardinia/Gippsland region, passionate about aged care and experienced in client and staff coordination.
Proudly owned and operated by Registered Nurses, we provide high-quality support services and specialised nursing care to help Gippsland community members live independently, with dignity, choice and comfort in their own homes.
In this role, you'll work closely with our Care Manager, collaborating with a supportive team to ensure our clients receive the very best care. You'll also help grow our client base, support our staff, and contribute to initiatives that make a genuine difference in people's lives.
Your key responsibilities will include:
* Overseeing the day-to-day care for your nominated clients
* Providing direct care and support where required
* Ensuring all care is delivered to the highest standard by skilled, caring staff
* Assisting with client growth and staff recruitment
* Supporting the Senior Care Manager to run the region smoothly and efficiently
* Building our reputation as the provider of choice in the community

What we're looking for:
* Experience in client coordination and aged care
* A proven ability to work collaboratively within a team
* Commitment to meeting the Aged Care Standards and legislative requirements
* EEN, Cert IV in Community Service, Aged Care or Individual Support or equivalent
* Current First Aid certificate and National Police Clearance
* Current Driver's Licence and a fully maintained, registered, and insured vehicle
* AHPRA registration (Required only for Nurses)
