About the Role:
Vacenti Aged Care is seeking a full-time Care Coordinator to join the clinical team at our Marebello Facility located at Victoria Point, QLD.
The successful candidate will report to and work closely with the Clinical Manager, supervising the day-to-day delivery of care and services to our residents.
They will provide clinical guidance and support to care and allied health staff, ensuring the delivery of person-centred care that meets the individual clinical, social, and emotional needs of our residents.
Main Responsibilities:
1. Provide clinical expertise and decision making to promote best possible nursing practice to residents.
2. Lead, coach, mentor and supervise registered nurses.
3. Be actively involved in delivering care and services to residents.
4. Participate in quality management systems of the home including data analysis and trends.
5. Promote and coordinate clinical documentation including assessment, care planning and evaluation, AN-ACC assessments and iCare documentation.
6. Check progress notes and other documentation daily.
7. Review and manage incidents when they occur.
8. Support residents, families and teams in coordinating case conferencing.
Requirements:
1. Relevant experience in senior nursing position.
2. Ideal candidate will have in-depth understanding of AN-ACC and quality assurance requirements that operate in aged care, along with sound supervision, organisational and communication skills.
3. Current registration with AHPRA required.
4. Must have right to live and work in Australia. If successful, will be required to obtain national police clearance prior to commencement.
