Assessor - Clinical Aged Care (Caboolture)

The role
As an Assessor – Clinical Aged Care you will enjoy conducting comprehensive assessments using the Integrated Assessment Tool (IAT) to evaluate and determine the needs of our customers seeking home support services. You will then develop personalised support plans and assist in the connection of relevant service providers, to help enable customers to retain their independence.
The role is offered as a permanent, full-time opportunity (Monday to Friday) with travel in the Caboolture region required in meeting with customers and stakeholders.
More about the role
1. Collaborating and communicating with customers, carers, families, representatives and other service providers, as appropriate, in the aged care planning process.
2. Undertaking and reviewing personalised support plans, effectively managing changing customer and carer needs with their consent, ensuring service recommendations are consistent with the Delegate decisions.
3. Identifying customers' formal and/or informal respite support and arrangements.
4. Developing an Emergency Care Plan if the customer does not have one in place.
5. Referring for services, assessing complexity and the risk of vulnerability, and assisting with linking support, reablement and short-term case management.
6. Participating in clinical supervision, and undertaking a variety of administrative functions, including report writing and correspondence preparation.
About you
We want our customers to have a great experience and as such you will have exceptional customer service skills and proven ability to form positive relationships with community members and stakeholders. You will also possess:
1. Tertiary qualification in a health-related discipline (e.g. Registered Nurse, Physiotherapist or Social Worker).
2. Current unrestricted AHPRA registration or part of, or eligible to be part of, a relevant professional association.
3. Minimum of one year demonstrated experience directly delivering services in aged care settings and/or to aged persons (such as geriatric evaluation, rehabilitation, palliative care, community nursing).
4. Knowledge or experience in conducting home assessments or risk assessments for vulnerable people, including the application of sensitivity for customers from diverse backgrounds and/or mental health conditions.
5. Demonstrated ability to communicate clearly and effectively with customers with hearing loss, other sensory loss and/or cognitive impairment.
6. Experience in undertaking assessments of customers with dementia who have responsive behaviours.
7. Adaptability and flexibility to be able to respond to changing task requirements in a positive way, and you embrace current and new technologies.
8. Willingness to use own registered and insured vehicle for work purposes, and a valid Driver's Licence.
9. Current First Aid and CPR Certificate, and evidence of the most current influenza vaccination.
