Job Description

The In-Home Care Coordinator plays a vital role in leading the coordination of tailored services that support clients to live independently and with dignity.

* Conduct initial assessments and service reviews to develop personalized care plans that meet client needs.
* Provide ongoing case management aligned with funding requirements, ensuring seamless service delivery.
* Coordinate referrals and ensure services meet client expectations.
* Lead and support In-Home Care Carers and Domestic Assistants, fostering a culture of collaboration and high-quality customer service.
* Participate in performance reviews, audits, and continuous improvement initiatives.

Key Responsibilities:

* Manage assessments, care planning, case management, and team leadership to deliver high-quality, individualized care that exceeds client expectations.
* Develop and implement care plans that cater to the unique needs of each client, promoting their independence and dignity.
* Cultivate strong relationships with internal stakeholders and external service providers to ensure efficient and effective service delivery.
