**Role Overview**: We are seeking a compassionate and motivated Disability Support Worker to join our dynamic team. In this role, you will play a vital part in providing high-quality support to participants, helping them achieve their personal goals and develop their independence. You will work closely with individuals to facilitate their community engagement, support their transition into adulthood, and build their capacity to live fulfilling lives.
**Key Responsibilities**:
- Provide person-centred support tailored to the unique needs and goals of each participant.
- Assist young people transitioning from out-of-home care and adults in building their independence and capacity.
- Facilitate community engagement and help participants access and navigate NDIS services.
- Foster a supportive and empathetic environment, respecting each participant's dignity and aspirations.
- Collaborate with other professionals, families, and carers to ensure comprehensive and effective support.
- Document and monitor participant progress, ensuring that all support provided is aligned with their individual plans.
- Promote and uphold the values of YNCA Pty Ltd, contributing to a culture of respect, empathy, and excellence in care.
**Qualifications and Skills**:
- Certificate III or IV in Disability, Community Services, or a related field (or equivalent experience).
- Previous experience in disability support or a related role is highly desirable.
- Strong communication and interpersonal skills, with a genuine passion for supporting and empowering individuals.
- Ability to work both independently and as part of a team.
- A commitment to delivering high standards of care with empathy and respect.
- Valid driver's licence and reliable vehicle (if required for the role).
