Home Care Packages Client Care Manager
We are seeking a skilled and compassionate Home Care Packages Client Care Manager to join our team.
About the Role
* This is an exciting opportunity to work with older adults, their carers, and families to provide high-quality care and support.
* As a Client Care Manager, you will use a person-centered approach to develop and coordinate services that meet the unique needs of each client.
Responsibilities
1. Develop, review, and monitor Care Plans using SMART goal methodology in consultation with clients on an ongoing basis.
2. Provide advice, support, assistance, and advocacy as requested by clients, carers, and families.
3. Act as the Primary Service Provider to direct the coordination and provision of services or support for clients and families.
Requirements
* Degree in relevant Social Welfare/Case Management or equivalent, with a minimum of 2 years' recent experience in Home Care Packages.
* Knowledge and skills in working with older adults, their carers, and families.
* Demonstrated experience in providing client support and case management services.
* Strong care planning skills, including sound time management skills.
* Solid understanding of SMART Goals.
