What you'll be doing
The Social Worker will be a member of the multidisciplinary Assertive Community Management (ACM) team in Drug Health Services, The social worker will provide comprehensive assessments such as psychosocial/family, Drug Health, psychological, functional and risk management assessments, evidence based interventions and services to clients of WSLHD under the direction of the ACM Team Leader and Director of Allied Health, Drug Health WSLHD. The Social Worker will promote the principles of harm mininisation, recovery and support to clients with a range of comorbid physical health problems and multiple psychosocial issues including accommodation and housing, social isolation, and vocational. This position will provide assertive follow-up for clients including home visits. This position is responsible for the provision of a high quality client model of care in relation to Drug and Alcohol dependence, perform specialist clinical services including comprehensive drug and alcohol assessments, develop and conduct timely periodic review of case management in collboration with consumers and their families, and assist clients in achieving their goals.

Essential Criteria
Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).
