THE ROLE
Our client seeks a Social Worker who wants to make a meaningful difference in the lives of patients navigating mental health and rehabilitation journeys. We are looking for proven experience in mental health and rehabilitation settings to provide on site psychosocial support and case management to patients and their families. This is a key role within their multidisciplinary team, offering a rewarding opportunity to contribute to holistic recovery and wellbeing.
THE ESSENTIALS
* Deliver psychosocial assessments, care planning, and counselling services
* Provide support across mental health, medical, and rehabilitation units
* Advocate for patients and coordinate discharge planning with internal teams and external agencies
* Support families through education, resources, and referrals
* Work across mental health, medical, and rehabilitation units as well as palliative care
* Collaborate closely with allied health professionals, medical staff, and community services
* Degree in Social Work and eligibility for membership with the AASW
* Experience in mental health and/or inpatient rehabilitation settings (hospital based experience highly regarded)
* Excellent interpersonal, communication and problem solving skills
* A collaborative approach with the ability to work autonomously and as part of a multidisciplinary team
* Sound understanding of the Australian healthcare and social services systems
