**Community Registered Nurse – Diabetes Specialist Nurse**

About the Role
As a member of our clinical community team, you will provide complex care to high acuity clients from various short-term/long-term programs. You will work both independently and as part of a team to deliver high-quality patient education and care in the comfort of a client's home.
Key Responsibilities
* Support and educate clients with Type 1 and Type 2 Diabetes.
* Educate and upskill staff to care for clients with Diabetes.
* Administer IV, IM, SC, and PO medications.
* Provide general nursing care, including simple and complex wound care, VAC dressings, IV therapy, IDC/SPC care.
* Accurately and timely document in all client records.
* Communicate effectively within the clinical team to ensure quality care.
* Complete initial assessments and develop care plans to ensure the best outcome for our clients.
* Monitor and update care plans to ensure appropriate care is provided.
* Deliver education to clients and families to enable the best possible outcome.
* Recognize and respond to clinical deterioration, with appropriate escalation.
* Promote best practice and initiate quality improvement activities.
What You Bring
* Current AHPRA Nursing registration.
* Minimum 2 years' acute nursing experience, with community experience desirable.
* Postgraduate qualifications in Diabetes Management and Education.
* NDIS Clearance (or willingness to obtain).
* Police Clearance (or willingness to obtain within 6 months).
* Use of own car with KM allowance provided.
* Full driver's license, reliable and comprehensively insured car.
* CPR and manual handling certification.
