* Communicate and liaise with clients' families, carers, and representatives to ensure appropriate support and continuity of care

We are seeking candidates with strong and flexible availability who are happy to travel across Rockingham, Fremantle and surrounding suburbs!

Skills, Attributes and Experience

The ideal candidate takes pride in delivering high-quality, person-centred care that respects and values each client's individual choices, goals, and independence. You will have strong interpersonal skills and the ability to build meaningful, long-lasting relationships with clients while maintaining professionalism and compassion. You will also demonstrate a positive attitude and a commitment to contributing effectively as part of a supportive home and community care team.

Key Criteria:

* Current NDIS Worker Screening Check and WA Working with Children Check, or willingness to obtain
* Proof of working rights in Australia (passport or valid visa)
* Valid driver's licence and access to a reliable vehicle
* Previous experience as a Support Worker in the health or disability sector or a relevant Certificate III qualification (highly desirable but not essential)
