* Located at Caulfield Hospital, Aged Care Assessment - Southern Metro Region
The Department has established a new Single Assessment System (SAS) to improve access to aged care services. As part of this system, Alfred Health Aged Care SAS provides comprehensive and home support assessments via the Victorian Dept of Health agreement.
This role will require you to travel to complete face-to-face assessments as well as telehealth and phone assessments as required. You will need to be adaptable to assessment demand and may be required to complete home support assessments.
Responsibilities
* Conduct thorough assessments of clients' needs and eligibility for aged care services
* Collaborate with interdisciplinary teams to develop and implement client-centered care plans
* Provide education and support to clients and their families on aged care services and options
You will be involved in training for the new Aged Care Act and Support at Home Program which commences in November. Experience And Qualifications Required: Degree or equivalent in a relevant Allied Health discipline or Bachelor of Nursing; registration with AHPRA; recent SAS experience and current compliance of all My Aged Care learning modules.
