About the Role
Key Responsibilities:
1. To assist clients with their daily needs including personal care assistance, mobility aid provision, transportation services;
2. To develop individualized care plans in collaboration with healthcare professionals and family members;
3. To administer medication as prescribed by medical practitioners;
4. To monitor health progress and report any concerns or changes to relevant authorities;
To facilitate social recreational activities that promote client engagement; maintain accurate records of progress ;and advocate for client rights within designated service settings. Role Expectations: The successful applicant must be able collaborate effectively, communicate respectfully, remain confidential always when handling sensitive information .
